Age Limit: Candidate should have not exceeding 37 years for Social Worker, Staff Nurse posts, not exceeding 32 years for Lab Assistant, Dietician, Pharmacist Allopathy, O.T. Assistant, ECG Technician and Occupational Therapist posts and 18-27 years for Jr. Radiographer, O.T. Technician, Dresser and Nursing Orderly posts as on 26-12-2012. (Age relaxation will be applicable as per rules).
Educational Qualification: Candidate must possess 10+2/ Matriculation/ Degree/ Diploma in relevant fields.
Application Fee: Candidates need to pay Fee Amount for Grade Pay 2401 and above is Rs.250/- (Rs.225/- + Bank collection charges Rs.25/-) for all other categories and Rs.150/- (Rs.125/- + Bank collection charges Rs.25) for Grade Pay 1800 to 2400/- (No fee for SC/ ST/ PWD/ Departmental Candidates, Female Candidates & Ex-servicemen) Remit the requisite fee through bank challan in Punjab National Bank in a specially opened account PNB, A/C 3247002100149692,PNB, Branch‐Ind Area, Phase ‐2, Chandigarh, on behalf of ESIC Model Hospital-Ramdarbar, Chandigarh.
Selection Process: Selection will be done on the basis of performance in written test and or interview.
How to Apply: Eligible candidates may apply through online from www.esicmhchd.org website from 05-12-2012 to 26-12-2012. Take two copies of print out for future reference and another copy of Online Application along with copies of other necessary documents should be sent through Registered Post/ Speed Post to the ESIC- Model Hospital, Industrial Area Phase‐II, Ramdarbar, Chandigarh 160002 on or before 31-12-2012.

1. Candidate have to visit ESIC website www.esic.nic.in/recruitment.php, and www.esicmhchd.org for filling the Online Application Registration.
2. Candidates are advised to read the instruction carefully “Guidelines to Candidates Option” given on ESIC website.
3. Fill the application carefully and submit the online application.
4. Before applying online a candidate will be required to have scanned (digital) image of his/ her photograph and signature.
5. Upload scanned Photograph, Signature and Thumb impression.
6. When the application is successfully submitted, a registration number will be generated by the system and displayed on the screen.
7. Candidates should print / note down the registration number for further reference.
8. Candidate will receive registration confirmation by E-MAIL after submitting the online application.
9. Candidates are advised to print the bank challan from website and submit the fee at any Punjab National Bank Branch and fill necessary details in system generated application form as per given space in bank details column.
10. And send system generated application form with necessary documents to the above mentioned address.
